Humana is selling the coverage year-round.
For consumers who meet the prior coverage requirement, the plan will cover basic and major dental services as well as ordinary preventive care. The definition of "basic services" includes extractions, root removal, fillings, space maintainers and prefabricated stainless steel crowns.
The plan provider network offers access to 270,000 dentists, and the plan also pays benefits for services received outside the provider network. For enrollees who get care out of network, the plan imposes a $50 deductible on individuals and a $150 deductible on families.
The annual maximum is $1,250 for an individual in the first year and $1,500 in later years.
Humana says the plan will be available in 21 states Alabama, Arizona, California, Colorado, Delaware, Florida, Georgia, Illinois, Indiana, Kansas, Kentucky, Louisiana, Minnesota, Missouri, North Carolina, Ohio, Pennsylvania, Tennessee, Texas, Utah, and Wisconsin.
