Sanitation over HIV fixation

Are aid agencies, NGO's, charities and governments working backward in Africa when it comes to the HIV, Malaria and TB epidemic? One chief in Zambia seems to think so. To him the prolific nature of the epidemic and the easiness with which the diseases spread is a result of poor sanitation. Should we be making sure villages have proper toilets before we vaccinate and hand out mosquito nets?
