Health care giant Humana is teaming up with Max-Wellness, provider of health products and services with locations in Florida and Ohio, to create a "geriatric care concierge service" known as Humana HomeCare Solutions. The program, which Humana calls the first of its kind, will help seniors age in place.

The program will provide seniors with access to nurse practitioners and other health care professionals, who will conduct comprehensive in-home analyses of medications and accessibility challenges. Once a senior's living situation has been evaluated, a nurse will devise a personalized care management plan and recommend the appropriate equipment or services to allow a senior to live independently. The home visit, consultation and management plan would cost $450.

Seniors do not have to be covered by Humana in order to take advantage of the HomeCare program, nor do they have to purchase recommended equipment from Max-Wellness. Furthermore, Max-Wellness has pledged that patients' medical information would not be used for marketing purposes by the retail division of the store.
