The Bend, Ore., chamber of commerce will be offering a single-state association health plan to any member employer with at least 2 full-time employees.

Clear One Health Plans Inc., Bend, Ore. (OTCBB:CCHN) is providing the Bend Chamber of Commerce plan coverage and Johnson Benefit Planning, Bend, is acting as the broker.

Chamber member firms that sign up for the program can choose from a menu of 4 medical plan options. The plans cover maternity care, mental health care and preventive care as well as major medical care.

Each employer group will be underwritten separately. Rates will depend on factors such as employer participation, employer contribution and use of wellness programs, organizers say.

The Bend chamber has about 1,300 member employers with about 25,000 employees.
