The Centers for Medicare & Medicaid Services has for the first time rated each of the nation's 15,800 nursing homes that participate in Medicare or Medicaid.

CMS assigns facilities star ratings ranging from a low score of 1 star to a high of 5 stars. It bases its scores on state health inspection surveys, staffing information and a variety of quality-of-care measures.

CMS said it developed the ratings after consulting with experts from academia and patient advocacy and nursing home provider groups.
