Florida Combined Life Insurance Company Inc. is introducing 2 fully insured preferred provider organization dental policies for individuals and families.

Florida Combined, a unit of Blue Cross and Blue Shield of Florida, Jacksonville, Fla., says the new BlueDental Choice Copayment and BlueDental Choice Plus policies will offer coverage comparable to that available through group dental plans.

Plan participants can enroll immediately and visit any dentist without having to select a provider in advance or obtain a referral, Florida Combined says.

The policies will protect members who visit in-network providers against balance billing, and members will not have to submit claim forms for services, Florida Combined says.

The choice copayment plan offers a network of 1,400 providers and a fixed out-of-pocket copayment costs for all in-network procedures

The choice plus plan offers a higher benefit level, access to 3,500 network providers, and a cost-sharing arrangement based on coinsurance percentages. Reimbursement for out-of-network care will be based on a percentile of "usual and customary" dental fees, Florida Combined says.
